This ol' boy is from 1922! :o Been mothballed since the '70s. It was my grandfathers, then my pops, and now mine. He resides in a safe with his other friends and discuss the years. :)

Any clue how to identify what barrel is what choke?

There should be a number stamped on the underside of each barrel in the flat area.
0 = cylinder
1 = improved
2 = modified
3 = improved modified
4 = full

Boy I tell you what, if you were here I would buy you a beer! Thank you.

My left barrel is full and right is modified. 30" barrels :D

Is the forward trigger left and rear trigger right?

The front trigger is the right barrel the back trigger is the left barrel. I believe this the thought process. As the flock of birds or pair of rabbits go past you the lighter choked barrel is used first because that is when they are closest to you. As the flock goes buy or you miss the rabbit they are now farther away and you will need the tighter choke to reach out that far.

That is exactly it. That's why 90% of all field guns were either ImpCyl/Mod or Mod/Full

Almost ALL double barreled shotguns are set up like that. Only a custom gun would be any different. The right trigger is the open barrel and the left the closed one.
